In the end F40PH 255 was deemed a total loss and scrapped along with MARC Cab Car 7752, as well as a few of the coaches on the front of the consist. P40DC 811 on the other hand was eventually repaired and returned to service but was put into storage in 2005 until being sold to LTEX in October 2019 where it is sitting to this day rotting away.
The crash occurred because the crew forgot about a caution signal as they approached the Kensington station for a station stop and then proceeded thinking the tracks ahead were clear, but ran a stop signal.
I saw the aftermath of the crash.....my ex worked in a building overlooking the crash site.....

I was operating VRE commuter trains Wash. DC to Fredericksburg, VA that evening. I found out what happened when I got to the crew room at Crossroads Yard. At the hearing they questioned the AMT engineer on what happened. His statement blows my mind to this day. When asked why he didn't put his train into emergency and stop, he replied oh no, that would have been suicide! The man asked why? He responded with, I decided right then and there I wasn't going into this running backwards, let's rock and roll baby, I wanted to punch that car as hard as I could, may sound kind of funny I guess. I almost fell down on the chair and had to read that statement 2 or 3 times. I was dumbstruck.
ALL ENGINEERS LEARN IN ENGINEER'S SCHOOL LEARN THAT "WHEN DELAYED WITHIN A BLOCK YOU MUST MAINTAIN RESTRICTED SPEED (LESS THAN 20 MPH) UNTIL THE NEXT SIGNAL'S INDICATION CAN BE CLEARLY SEEN!!!" THE CREW OF THE MARC FAILED TO DO THIS BECAUSE THEY FORGOT THE INDICATION THEY WERE OPERATING ON, OR NEVER SAW IT IN THE FIRST PLACE?
That crash was the result of a S.P.A.D signal passed at danger
