Usually an investigation is done to ensure  a thorough analysis of what could have happened, mitigate risk in future and safety measures  are put in place to ensure this never recurs. My fear is that history will repeat itself unless we learn from this disaster. An act of God doesn’t take away the fact that the rail had disengaged. Rains are a constant occurrence. What will stop these happening again? I think this interview should go beyond the personal to accountability by government and relevant authorities to review systemic issues causing these accidents.
