Just taking a guess, but I'm assuming that thing at the end was a "Damsel". The thing supposedly some birds got turned into. The thing that would have snatched the unarmored guy if he tried going outside. Judging by the statement of "Damsel season" towards the end, it may have been years since the flashes. Long enough to give them a sort of time period of activity that the "people" can recognize.

so, does it mean they both wanted to die as humans, so that's why the exposed guy didn't go outside to be snatched by these mosquito things? was the "surgeon" their only way to pass away in the somewhat human form? if yes, then the ending really is soul crushing😞

This reminds me of Nightmare on elm street when the guy became fused to his street bike. This would be the craziest circle of hell in my opinion
